레지오넬라 뉴모필라 감염병, Legionella pneumophila Infections

 

역학, 원인

  • 레지오넬라 뉴모필라(Legionella pneumophila)균에 감염되어 생기는 감염병을 레지오넬라증(Legionellosis) 또는 레지오넬라 뉴모필라 감염병이라고 한다
  • 사람에서 사람으로 감염되지는 않는다.
  • 거의 80% 이상의 레지오넬라 질환은 L pneumophila 균이 오염되어있는 가정용 물병원직장공공시설 등에 있는 분무수에어콘디션가습기 등에서 나오는 분무 수를 흡입하거나병원이나 호텔 등에서 사용하는 에어콘디션을 통해서 감염될 수도 있다.
  • 건강하고 면역 적격 소아들에게 이 병이 생기면 무증상이든지 경도로 앓거나 이런 병을 앓는지 모르고 지날 수 있다.
  • 그러나 면역결손이나 악성 종양 치료를 받거나 심한 면역결핍증이 있거나 ,만성 말기 신장병이 있거나 기관 이식치료를 받거나 폐병이 있거나, 면역 억제제 치료를 받거나 전신 스테로이드 치료를 받는 아이가 이 병에 걸리면 심하게 앓을 수 있다.

잠복기

  • 레지오넬라 질환의 잠복기는 2~10일이고 폰티악 열(Pontiac fever)의 자복기는 1~2일 이다.

증상 징후

  • 레지오넬라증(Legionellosis)에는 레지오넬라 질환(Legionnaires disease)와 폰티악 열(Pontiac fever) 등 두 종류의 감염병이 있다.
  • 레지오넬라 질환에 걸리면 경도 내지 중증도 폐렴이 생길 수 있고 그로 인해 열기침진행성 호흡 곤란증한전근육통 등의 증상 징후가 나타날 수 있다.
  • 그리고 위장 장애중추신경 장애신장 장애 등도 생길 수 있다. 그런 장애로 인한 증상 징후도 나타날 수 있다심하면 호흡 부전증도 생기고 그로 인해서 사망도 한다.
  • 폰티악 열(Pontiac fever)에 걸리면 폐렴은 생기지 않지만 갑자기 플루의 증상과 비슷한  증상-열, 두통, 허약,근우통 등이 생길 수 있다.

진단

  • 병력 증상 징후 진찰 소견을 종합해서 이 병을 의심하면 호흡기 분비물폐 생체조직늑막 강 액 등에서 얻은 피 검물로 세균 배양 검사를 해 Legionella 세균을 검출 진단 한다.
  • 소변 항원 검사직접 면역 형광 분석 검사, PCR 검사, 항원 항체 검사, IFA, Legionella DNA 검사 등으로 진단한다.

치료

  • 레지오넬라 질환에 걸리면 Azithromycin, Erythromycin, Levofloxacin, Fluoroquinolone, Doxycycline, Trimethoprim-sulfamethoxazole, Tetracyclines 등 항생제 중 한 종류의 항생제로 치료한다.
  • 폰티악 열(Pontiac fever)에 걸리면 항생제로 치료하지 않는다.

Legionella pneumophila Infections

Epidemiology, causes

• Infectious diseases caused by infection with Legionella pneumophila are called Legionellosis or Legionella pneumophila infectious diseases.

• There is no human-to-human transmission.

• More than 80% of Legionella diseases are caused by inhaling household water contaminated with L pneumophila, spray water from hospitals, workplaces, public facilities, etc., spray water from air conditioners, humidifiers, etc. It can also be infected through

• When the disease develops in healthy, immune-competent children, it may be asymptomatic or mildly ill or pass without knowing that they have it.

• However, a child who has been treated for an immunodeficiency or malignant tumor, has severe immunodeficiency, has chronic end-stage renal disease, has undergone organ transplantation, has lung disease, is on immunosuppressant therapy, or is receiving systemic steroids can become seriously ill. . incubation period

• The incubation period for Legionella disease is 2 to 10 days, and the incubation period for Pontiac fever is 1 to 2 days.

Symptoms, Signs

• Legionellosis includes two types of infectious diseases: Legionnaires disease and Pontiac fever. • Legionella disease can cause mild to moderate pneumonia, which can cause symptoms such as fever, cough, progressive dyspnea, fever, and muscle pain.

• Gastrointestinal disorders, central nervous system disorders, and kidney disorders may also occur. Symptoms of such disorders may also be present. In severe cases, respiratory failure can occur, which can lead to death.

• Pontiac fever does not cause pneumonia, but flu-like symptoms—fever, headache, weakness, and muscle cramps can occur suddenly.

Diagnosis

• If the disease is suspected based on the history, symptoms, signs, and examination findings, a bacterial culture test is performed with blood samples obtained from respiratory secretions, lung tissue, and pleural cavity fluid to detect and diagnose Legionella bacteria. • Diagnosis is made by urine antigen test, direct immunofluorescence assay, PCR test, antigen antibody test, IFA, and Legionella DNA test.

Cure

• If you get Legionella disease, treat it with one of antibiotics, such as Azithromycin, Erythromycin, Levofloxacin, Fluoroquinolone, Doxycycline, Trimethoprim-sulfamethoxazole, or Tetracyclines.

• If you have Pontiac fever, do not treat it with antibiotics.
