사람 폐 포자충 감염병, Pneumocystis jirovecii Infections

 

역학

  • 폐 포자충()은 포유동물들의 폐 포자충 감염병을 일으킨다특히 쥐에 감염되어 쥐 폐 포자충증을 일으킨다.
  • 미숙아나 영양실조에 걸린 영아들이 드물게 폐 포자충 폐렴에 걸릴 수 있다.
  • 그러나 HIV 감염으로 면역손상이 생기면 이 병이 흔히 생길 수 있다그 외 어떤 원인으로 면역 손상이 있는 아이들이나 고형 기관 이식수술 치료 수혜자암 치료를 받는 자선천성 면역 결핍증이 있는 아이들에게 폐 포자충 폐렴이 더 잘 생길 수 있다.

원인

  • 폐 포자충(Pneumocystis은 진군(곰팡이)의 일종이다.
  • 사람 폐 포자충 Pneumocystis jirovecii과 쥐 폐 포자충 Pneumocystis carinii 등이 있다.

잠복기

  • 확실히 모른다.

증상 징후

  • 건강한 사람들이 폐 포자충에 감염되어도 증상이 드물게 나타난다그러나 면역 손상이 있는 병에 걸려 앓는 영아들이나 유아들에게 폐 포자충에 감염되어 폐 포자충 폐렴에 걸리면 숨이 가쁘고빈박저산소증이 생기고가래가 나오지 않는 기침을 하고열 등이 나는 호흡기 감염병의 증상이 나타날 수 있다.
  • 면역 손상이 있는 아이들의 폐 포자충 폐렴은 급성으로 또는 전격적으로 발병할 수 있다.
  • 폐 양측이 광범위 폐렴 영상이 생기고 또는 속립성공동성결절성 등의 폐 포자충 폐렴 영상 소견이 나타날 수 있다.
  • 폐 포자충 폐렴을 적절히 치료해도 5-40%의 사망률이 생길 수 있고 치료를 받지 않으면 거의 다 죽는다.

진단

  • 병력+증상 징후+진찰소견+임상검사로 진단한다폐 조직이나 호흡기의 분비물 피검물로 특별 염색 현미경 검사를 해서 폐 포자충 낭을 발견해 진단한다. PCR 반응분석 검사로도 진단한다.

치료

  • TMP-SMX, Pentamidine, Atovaquone, Corticosteroid 등으로 치료한다.

예방

  • 화학 예방요법은 상당히 효과가 있다.
  • HIV 감염병으로 상당한 면역손상이 있거나 선천적 또는 후천적 세포매개 면역손상이 있는 사람들은 폐 포자충 폐렴에 잘 걸릴 수 있다이런 경우 TMP-SMX 화학요법은 상당한 효과가 있다조혈 줄기세포 이식고용기관 이식치료백혈병림프종 코티코스테로이드 치료를 받는 아이들은 면역손상이 생길 수 있고 그들은 폐포자충 폐렴에 걸리기 쉽다따라서 화학예방 요법으로 치료를 한다.
  • 주로 TMP-SMX로 예방한다교대용으로 Atovaquone이나 Dapsone로 예방 치료한다.

그 외

  • 폐 포자충 폐렴으로 입원한 환자는 격리치료를 받는다예방접종 백신은 없다.

 

Human Pneumocystis Infections, Pneumocystis jirovecii Infections

Epidemiology

• Pneumocystis (species) causes pneumocystisiasis in mammals. In particular, it infects mice and causes mouse pneumocystisiasis.

• Premature or malnourished infants may rarely contract Pneumocystis pneumonia. • However, HIV infection can cause the disease to be common if the immune system is compromised. Children with immunocompromised causes of any other cause, recipients of solid organ transplant surgery, those receiving cancer treatment, or children with congenital immunodeficiency are more likely to develop pneumocystis pneumonia.

Cause

• Pneumocystis (species) Pneumocystis is a kind of fungus.

• Pneumocystis jirovecii in humans and Pneumocystis carinii in mice. Latency

• Not sure.

Symptoms Signs

• Even healthy people infected with Pneumocystis have rare symptoms. However, when infants and young children who are ill with an immune-compromised disease are infected with Pneumocystis and contracted with Pneumocystis pneumonia, symptoms of respiratory infections such as shortness of breath, frequent heartbeat, hypoxia, coughing with no sputum, fever, etc. may appear.

• Pneumocystis pneumonia in immunocompromised children can develop acutely or fulminantly.

• Extensive pneumonia images may occur on both sides of the lungs, or images of pneumocystis pneumonia such as miliary, cavitary, or nodular may appear.

• Even with adequate treatment of Pneumocystis pneumonia, a mortality rate of 5-40% can occur and almost all die without treatment.

Diagnosis

• Diagnosis is made by medical history + symptoms signs + examination findings + clinical examination. Pulmonary sporozoite cysts are detected and diagnosed by special staining microscopic examination of lung tissue or respiratory secretion specimens. It is also diagnosed by PCR reaction analysis.

Treatment

• Treat with TMP-SMX, Pentamidine, Atovaquone, or Corticosteroid. Prevention

• Chemoprevention is very effective.

• People with significant immunocompromise due to HIV infection or with congenital or acquired cell-mediated immunocompromise may be more susceptible to Pneumocystis pneumonia.

In this case, TMP-SMX chemotherapy is very effective. Children receiving hematopoietic stem cell transplantation, organ transplantation therapy, leukemia, lymphoma, and corticosteroid therapy may develop immune compromise and they are susceptible to Pneumocystis pneumonia. Therefore, it is treated with chemopreventive therapy.

• Prevention is mainly with TMP-SMX. Prophylactic treatment with Atovaquone or Dapsone as an alternative. Others

• Patients hospitalized for Pneumocystis pneumoniae receive isolation treatment. There is no immunization vaccine.
