샤가스 병 (아메리카 파동편모충증), Chagas disease(American trypanosomiasis)

 

역학. 원인

  • 샤가스 병은 크루스 파동 편모충 원충(Trypanosoma cruzi)이라는 원충( Protozoa) 전염병이다.  이 원충에 감염된 키스 버그(kissing bugs) 똥이 피부나 눈이나 입 점막에 접촉되면 이 기생충 병에 걸일 수 있다.
  • 선천적으로도 감염될 수 있고, 수혈, 고형기관 이식, 감염된 음식물 섭취, 주사 바늘을 통해서도 이 병에 걸일 수도 있고, 임상 실험실 요원이 감염된 기구 접촉으로 감염될 수 있다.
  •  미국에서 주로 외국으로부터 수입 감염이 되나 래쿤이나 설치류를 통해서도 감염될 수 잇다.,
  • 샤가스 병을 아메리카 파동 편모충증 또는 아메리카 수면증(Chagas Disease/American  Chagas Disease, American Trypanosomiasis 이라고도 한다.

잠복기는 1-12주이다. 그 보다 몇 년이 될 수도 있다.

증상 징후

  • 2~3개월 동안 급성기를 지나 만성기로 이어진다
  • 발병후 3주 정도 되면 뇌막 뇌염에 걸일 수 있다.
  • 즉 샤가스 병은 급성 샤가스병과 만성 샤가스 병으로 분류된다.
  • 적절히 치료를 받지 않으면 일생동안 이 병으로 앓든지 사망한다.
  • 급성 샤가스병은 2~3 개달 동안 계속되고
  • 혈관 내 기생충이 발견될 수 있다. 이떄 무증상이거나 일정치 않은 증상이 생길 수 있다. 이 원충이 체내로 침입할 때 생긴 침입 피부 환부(보통 얼굴이나 팔 부위)가 부을 수 있다. 이때 생긴 환부를 영어로 Chagoma라고 한다.  편칙성 안검이 붓을 수 있다. 이런 징후를 영어로 Romana 징후라고 한다.
  • 때로는 결막염과 동칙성 귀앞 림프절 비대, 열, 권태, 간 비장 비대,  뇌염혈전증거대 식도거대 결장급성 심근염, 등이 생길 수 있다.
  • 급성 샤가스 병이 아급성이나 만성으로 이어지면서 아무 증상이 없을 수도 있고 때로는 각종 심장 병, 소화기 병 등이 생겨 환자가 황패해지고 생명을 잃을 수도 있다.
  • 면역 손상이 있는 사람에게는 이 병이 생기면 훨씬 심하게 앓을 수 있다.

진단

  • 병력, 증상 징후, 진찰 등을 통해 이 병이  있다고 추정진단이 나오면 급성기 피 피검물로 병원체를 검출 해 확진 한다.
  •  라틴 아메리카 등 지역에 여행한 적이 있는지 수혈을 받은 적이 있는지모체가 이런 병을 앓은 경험이 있는지 등등의 병력을 참조하고 IFA, ELISA, Wesrern blot 검사, IgA 항체 검사, PCR 분석 검사 등으로 진단한다.

치료

  • Nifurtimox 이나 Benznidazole로 치료한다.

Chagas disease (American trypanosomiasis)

Epidemiology. cause

• Chagas disease is a protozoa contagious disease called Trypanosoma cruzi. If the feces of kissing bugs infected with this parasite come into contact with the skin or the mucous membranes of the eyes or mouth, they can become infected with this parasitic disease.

• The disease can be transmitted congenitally, through blood transfusions, solid organ transplantation, ingestion of infected food, needles, and by clinical laboratory personnel through contact with infected instruments.

• In the United States, infection is mainly imported from foreign countries, but it can also be transmitted through raccoons or rodents.

• Chagas disease is also called Chagas Disease/American Chagas Disease, American Trypanosomiasis. The incubation period is 1-12 weeks. It could be several years older than that.

Symptoms, Signs

• Acute phase followed by chronic phase for 2-3 months

• You can develop meningoencephalitis about 3 weeks after the onset.

• Namely, Chagas disease is classified into acute Chagas disease and chronic Chagas disease.

• If not properly treated, the disease will last a lifetime or die.

• Acute Chagas disease lasts 2-3 months and

• Vascular parasites may be found. In this case, asymptomatic or inconsistent symptoms may occur. Invasive skin lesions (usually the face or arms) that form when these protozoa enter the body may swell. The affected area is called chagoma in English. Anomalous eyelids may swell. These signs are called Romana signs in English.

• Occasionally, conjunctivitis and sinus anterior lymph node enlargement, fever, malaise, hepatomegaly, encephalitis, thrombosis, giant esophagus, megacolon, acute myocarditis, etc. may occur.

• As acute Chagas’ disease becomes subacute or chronic, there may be no symptoms, and sometimes various heart and digestive diseases may cause devastation and death of the patient.

• People with compromised immune systems can get much worse with this disease. Diagnosis

• If a presumptive diagnosis of the disease is made through medical history, symptom signs, and examination, the pathogen is detected and confirmed with the acute stage blood specimen.

• Diagnose with IFA, ELISA, Wesrern blot test, IgA antibody test, PCR analysis test, etc. with reference to medical history such as travel to Latin America or other regions, transfusions, mother’s experience with these diseases, etc.

Treatment

• Treat with Nifurtimox or Benznidazole.
