RS 바이러스 감염병의 원인, 증상 징후

RS 바이러스를 호흡기 세포융합 바이러스, 호흡기 합포체 바이러스, Respiratory syncytial virus, 또는 RSV라고 한다. 1956년에 처음 확인 판명된 바이러스이다.
생후 1세가 되기 전 50%의 신생아들과 영아들이 이 바이러스에 감염된다.
만 2세가 되기 전 영아들이 2차나 그 이상 RS 바이러스에 감염된다.
세기관지염의 50%, 병원 외 폐렴의 44%, 병원 내 폐렴의 63%의 원인은 RS 바이러스이다.
겨울철과 봄철에 더 유행한다.
미국에서는 연간 126,300명의 영아들이 RS 바이러스 감염병으로 병원에 입원한다.
세계적으로 연간 1백만 명이 RS 바이러스 감염병으로 죽는다(출처; Consultant for pediatricians, December 2008, vol.7 NO. 12).
영유아기 이후 유아기·학령기·사춘기 아이들이나 성인들이 RSV에 감염되면 주로 상기도에 감염되고 그로 인해 콧물, 기침, 열 등의 증상 징후가 생기고 감기 비슷하게 앓는 것이 보통이다.
생후 첫 몇 주 신생아들이나 영아들이 RSV에 감염되면 경미한 상기도염의 증상 징후가 생길 수도 있고 하기도에 심하게 감염되면 급성 RSV 세기관지염 또는 RSV 폐렴이 생길 수 있다.
RS 바이러스 세기관지염 또는 RSV 폐렴이 있을 때 호흡곤란이 생기면서 콧방울(비익)가 벌렁거리고, 앞가슴 벽이 흉강 속으로 빨려 들어갔다 나왔다하고, 빈박증, 청색증이 생기고, 천명이 나면서 아주 심하게 앓을 수 있다.
기민 상태에 빠질 수 있고 신경이 예민하고 식음을 전폐하기도 한다.
선천성 심장염이 있는 미숙 신생아들이나 면역체 결핍증이 있는 영유아들, 또는 폐성 고혈압이 있는 아이들이 RSV에 감염되면 생명을 위협받을 수 있을 정도로 심히 앓을 수 있다.
RSV는 사람에게만 감염되는 바이러스이다.
RSV에 감염되어 앓는 환아나 보균한 사람이 호흡할 때 호흡기를 통해 나온 비말 속 RSV에 감염 될 수 있다.
사람에게 직접 근접하거나 호흡할 때 나온 비말 속 RSV에 감염된다.

RS 바이러스 감염 진단

병력, 증상 징후, 검진 결과를 종합하고 면역 형광 검정법이나 효소 면역학적 검정법 등 속성 진단법으로 진단할 수 있다.
면역 형광 검정법이나 효소 면역학적 검정법의 진단 특이성은 53~96%이다.
바이러스 배양검사로 확진할 수 있다.

RS 바이러스 감염병 치료

대증 치료를 하고
심하게 앓을 때는 Ribavirin 등 항바이러스제로 치료할 수 있다.
생후 24개월 이하이고 어떤 건강문제가 있고 건강상 고위험군에 속하는 영유아들은 면역글로불린과 Palivizumab로 RSV 감염병을 예방할 수 있다.
